RAPID RECOVERY MOBILE IV TERMS OF SERVICE

These Terms of Service ("Terms") govern your use of mobile intravenous therapy services provided by Rapid Recovery Mobile IV ("we," "us," or "our") in Scottsdale, Arizona, and surrounding areas. By scheduling an appointment or receiving our services, you agree to these Terms.
1. SERVICES
Rapid Recovery Mobile IV provides mobile intravenous (IV) hydration and vitamin therapy services delivered by licensed medical professionals to clients in their homes, offices, or other designated locations within our service area.Service Area: We exclusively serve Paradise Valley, North Scottsdale, McCormick Ranch, Arcadia, and Fountain Hills.
Service Providers: All IV therapy treatments are administered by licensed Registered Nurses (RNs) or Licensed Practical Nurses (LPNs) under the supervision and orders of a licensed physician in compliance with Arizona Board of Nursing regulations.
Medical Oversight: All treatments require a valid medical order from our supervising physician, which may require a Good Faith Examination (GFE) in accordance with Arizona regulations.
2. ELIGIBILITY FOR SERVICES
To receive our services, you must:
- Be at least 18 years of age
- Complete our health history questionnaire truthfully
- Undergo a pre-treatment assessment by our medical staff
- Meet our medical screening criteria
- Not have any contraindications to IV therapy
- Provide informed consent for treatment
We reserve the right to refuse service to anyone if our medical professionals determine that IV therapy may not be appropriate or safe based on your health status or medical history.
3. SCHEDULING AND CANCELLATION
Appointments: Services are available by appointment only, subject to availability.
Cancellation Policy: We require at least 1 hours' notice for cancellations. Cancellations with less than 1 hours' notice may be subject to a cancellation fee of 50% of the scheduled service price.
Late Arrivals: If you are not available within 15 minutes of your scheduled appointment time, we may need to reschedule your appointment and a cancellation fee may apply.
4. PAYMENT TERMS
Pricing: Current pricing for all services is available on our website or by contacting our office. Prices are subject to change.
Payment Methods: We accept major credit cards, cash, and certain mobile payment applications. Payment is due at the time of service.
Insurance: We do not bill insurance directly. Upon request, we can provide you with documentation that you may submit to your insurance provider, but we make no guarantees regarding reimbursement.
5. HEALTH RISKS AND INFORMED CONSENT
Risks: IV therapy carries inherent risks, including but not limited to infection, bruising, pain, inflammation, vascular injury, allergic reaction, and infiltration. These risks will be explained in our Informed Consent document.
Informed Consent: Prior to receiving treatment, you must read and sign our Informed Consent form, acknowledging your understanding of the potential risks, benefits, and alternatives to IV therapy.
Medical Advice: Our services are not a substitute for emergency medical care or your primary healthcare provider. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
6. LIMITATION OF LIABILITY
To the maximum extent permitted by law, Rapid Recovery Mobile IV and its owners, employees, contractors, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits or revenues, whether incurred directly or indirectly, or any loss of data, use, goodwill, or other intangible losses resulting from:
- Your use of or inability to use our services
- Any unauthorized access to or use of our servers and/or any personal information stored therein
- Any interruption or cessation of transmission to or from our services
- Any bugs, viruses, trojan horses, or the like that may be transmitted to or through our services
7. INDEMNIFICATION
You agree to defend, indemnify, and hold harmless Rapid Recovery Mobile IV and its owners, employees, contractors, and agents from and against any claims, liabilities, damages, losses, and expenses, including without limitation reasonable attorney's fees and costs, arising out of or in any way connected with your access to or use of our services, or your violation of these Terms.
8. GOVERNING LAW
These Terms shall be governed by and construed in accordance with the laws of the State of Arizona, without regard to its conflict of law provisions. Any legal action or proceeding relating to these Terms shall be brought exclusively in the state or federal courts located in Maricopa County, Arizona.
9. SMS/Text Messaging Terms & Conditions
Program Description: RAPID RECOVERY MOBILE IV (“we,” “us,” or “our”) offers a mobile messaging program (the “Program”), which you agree to use and participate in subject to these SMS Terms & Conditions. By enrolling in or otherwise agreeing to participate in the Program, you accept and agree to these Terms & Conditions.
User Opt-In: When you provide your mobile phone number and opt in (e.g., by checking an opt-in box or replying “YES” to our message), you consent to receive promotional and/or transactional text messages from us. These messages may include promotions, reminders, alerts, and responses to your inquiries. Message frequency may vary and may depend on your interactions with us.
Message and Data Rates Message: and data rates may apply to each text message sent or received, as provided in your mobile telephone service rate plan. Please consult your wireless carrier for applicable fees and charges. We do not charge you for sending or receiving text messages through the Program.
How to Opt Out: If you wish to stop receiving text messages from us, you can reply STOP to any SMS we send. You will receive one final confirmation message stating that you have been unsubscribed. No further messages will be sent unless you re-enroll by opting in again.

Participation Requirements: You must have a wireless device with text messaging capability, use a participating wireless carrier, and provide a valid mobile phone number. Not all carriers may support our service.
Privacy: We respect your privacy. We will only use the information you provide in connection with the Program as described in these Terms & Conditions and in our Privacy Policy. We do not sell, rent, or lease your personal information to third parties.
Disclaimer of Warranty: We make no warranties, express or implied, regarding the performance of the Program. We are not responsible for delayed or undelivered messages.
Modification of Terms & Program: We reserve the right to cancel or modify these SMS Terms & Conditions or discontinue the Program at any time. In the event of any such change, we will post revised Terms & Conditions to our website. Your continued participation after changes are posted constitutes acceptance of the updated terms.
10. CHANGES TO TERMS
We reserve the right to modify these Terms at any time. We will provide notice of any material changes by updating the "Last Updated" date at the top of these Terms. Your continued use of our services after such changes constitutes your acceptance of the new Terms.
